Please include a photocopy of your:

  • Merchant Mariner’s Document (pages 1 - 6)
  • STCW 95 Basic Safety Course Certificates
  • STCW 95 United States Coast Guard Endorsement
  • All licenses and training documents, including GMDSS
  • Information page of your current passport
  • Drug Free Certificate (if less than six months old)
  • Transportation Worker's Identification Credential (TWIC)

Please Note:
1. WHOI requires all crew members to have United States Coast Guard licenses and endorsements.
2. You must also have a Transportation Workers Identification Credential (TWIC).
