Network Connection - Move

The Network Connection Move form is used to activate ports if you are moving to a new office. To receive a new IP address for your new location, log in to WHOI Netreg or contact the CIS Helpdesk. Please submit requests as early as possible as most requests take up to 3 days to complete. For further information, please contact the CIS Helpdesk.

Reminder: check the new office for active jacks before using this form.
