II. Please refer to the documents listed below when completing the performance evaluation. The guidelines and evaluation forms have been revised and standardized and are to be used throughout the Institution for the evaluation of all Non-Exempt staff and Exempt Administrative Staff. The documents are:
III. It is important that all employees be informed of their job performance. The evaluation forms have a place for the employee to sign-off and to indicate any comments, if desired.
IV. Completed evaluation forms are to be sent to the Human Resources Office to be maintained in the employee's personnel file. The employee may receive a copy of the evaluation form if requested
V. Members of the Scientific and Technical Staff are to complete an annual Staff Information Sheet prepared by their department and submit that document to their respective Department Chairs.
