Merchant Mariner’s Document (both sides)
STCW 95 Basic Safety Course Certificates
STCW 95 United States Coast Guard Endorsement
All licenses and training documents, including GMDSS
Information page of your current passport
Drug Free Certificate (if less than six months old)

Please Note:
1. WHOI requires all crew members to have United States Coast Guard licenses and endorsements.
2. You must also have applied for a Transportation Workers Identification Credential (TWIC).
