Network Jack Activation - Additional

The Network Additional Jack Activation form is used to have one or more Ethernet jacks activated. These services are for all workstations, printers, and servers. To request a new IP address for your new system, log in to WHOI Netreg or contact the CIS Helpdesk. Please submit requests as early as possible as most requests take up to 3 days to complete. For further information, please see the New User Guide and/or contact the CIS Helpdesk.

Please remember that small Netgear 4-port switches are sold in the Stockroom for about $50.
The charge for activating a jack is $235.
