Dependent Life Insurance
You may purchase Voluntary Life Insurance coverage for your spouse (or domestic partner -- affidavit required) and unmarried dependent children.
For dependent child(ren) you may cover each child for $2,000 or $5,000 up to age 26.
Spousal insurance is available in increments of $5,000, up to a maximum of $100,000. If electing spousal life insurance for the first time during your 'initial eligibility' and the amount exceeds $30,000, you must provide proof of good health by completing the medical "Evidence of Insurability Form". Additionally, if your spouse is also a WHOI employee, you cannot elect to cover they under spouse life insurance. In addition, if you elect coverage or increase coverage outside your initial eligibility period, you must then complete "Evidence of Insurability" regardless of the amount of coverage. Spousal insurance may be covered up to age 70.
NOTE: Your initial eligibility is defined as your official employment date into a regular benefited position.
