Voluntary Accidental Death & Dismemberment Insurance

You have the option of purchasing voluntary accidental death and dismemberment insurance for accidents that occur on or off the job, including while traveling to and from work. You may purchase coverage for yourself alone or yourself and your eligible dependents. Coverage is available in $10,000 increments, starting at a minimum of $10,000 up to a maximum of $500,000.

Eligible dependents include your spouse (or domestic partner - affidavit required) and unmarried children up to age 19 (or up to age 25 if a full-time student and primarily supported by the employee).  Spousal insurance may be covered up to age 70.
