Please complete one of the following forms to request work on a telephone, including new telephone requests, telephone moves, extension changes, voice mail box changes and/or additions. For additional information, please contact Telecommunications.

The forms below are only accessible to computers that are on the WHOI network. If you need to request telephone work from outside WHOI and cannot connect via VPN, please contact the CIS Helpdesk at extension 2439.
